Medical Case Management
Medical Case management (MCM) has been essential to many people living with HIV. South Carolina HIV/AIDS Council has provided Medical Case Management for over 7 years. This program has helped people access medical care, housing, emergency financial assistance, food, clothing, emotional support, mental health/substance abuse treatment programs and the AIDS Drug Assistance Programs (ADAP).
Medical case management is funded by the Ryan White CARE Act. In order to qualify for any Ryan White-funded service, an individual must first provide documentation proving that they are HIV positive, their county of residence and their income. It is essential that potential clients bring proof of these three qualifiers with them when they present for case management.
The primary goal of Ryan White-funded case management is to promote access to health care, so the standards require consumers to be actively enrolled in primary medical care. For adults, active enrollment is defined as at least one visit with a primary care provider every six months. MCM’s main goal is to enroll clients in medical case management help them maintain their medical care.
Another essential service added to Medical Case Management is Peer Health Navigator (PHN). The role of the PHN is to help clients navigate through the system for medical care. PHN will assist in medical transportation, treatment adherence counseling, and other secondary support needs.
